Medication

Any medication (Tylenol, Ibuprofen, Midol, cough drops) to be administered at school MUST BE BROUGHT in a new container by the parents or student with the completed and signed form. Medication authorization forms and health care plans are renewed EACH school year. A separate medication authorization form is required for EACH medication brought to school by the parent.

Individual Health Care Plans

Individual health care plans are required for all students with asthma, diabetes, brittle bone disease, severe food or insect allergies necessitating availability of Epi-pens, etc. If your child has severe food allergies or celiac disease and requires special dietary measures, you must have your doctor complete and have on file at BMS the special dietary form. Parents are responsible for providing the updated yearly Individual Health Care Plan to the school. All necessary forms are available in the school office or on the BMS website.

Transfer Students

Transfer students must show immunization documentation when registering at BMS. Sixth grade  students must show documentation for the Tdap booster immunization required by the VDOE and VA State Law. Parents of transfer students are welcomed to visit the clinic for clarification of any health concerns or policies that may be different form the previous school district.

 Screenings

Screenings for vision, hearing and scoliosis are mandated by VDOE for all 7th graders. The screenings have been delayed until mid-October and will take place during PE. Please review the scoliosis fact sheet on the BMS website under Clinic Forms. If you have questions, call your health care provider or the clinic. The scoliosis opt out form is also in that section and must be completed and returned to the nurse for your child to not be screened. You may write on the scoliosis form if you want other screenings omitted. The scoliosis parent letter is also available on the BMS website under Clinic Forms.
