ANY medication (Tylenol, ibuprofen, Midol, cough drops) to be administered at school MUST BE BROUGHT in a new container by the parents or student with the completed and signed form. Medication authorization forms and health care plans are renewed EACH school year. A separate medication authorization form is required for EACH medication brought to school by the parent. Individual health care plans are required for all students with asthma, diabetes, brittle bone disease, severe food or insect allergies necessitating availability of epipens, etc. If your child has severe food allergies or celiac disease and requires special dietary measures, you must have your doctor complete and have on file at BMS the special dietary form. All necessary forms are available in the school office or on the BMS website. Parents are responsible for providing documentation that all required immunizations are up-to-date, completed, or a medical or religious exemption has been obtained. If you have questions please see the school nurse, Glenna Housman, at registration. She will have a table set up in the gym.
