If you are a new student to the area and are planning to enroll here at Altavista Combined School, please download and complete the three forms below. Please submit the completed forms to our Guidance Office between 8 - 3 pm. These forms are not necessary for rising 6th graders who attended Altavista Elementary School during the 2010-2011 school year.
