Clinic Notes
Medication
All medications including Tylenol, Ibuprofen, and any other over the counter medication along with prescription medicines must be brought to the clinic by a parent. Parents must also fill out a Medical Authorization Form and submit this form to Mrs. Wall. No student can be administered medication unless he or she has their own medication and an Authorization form on file. Click here for the Medical Authorization Form.
Individualized Health Care Plans
All students who have a chronic medical condition such as Diabetes, Asthma, severe allergies, seizures, etc., must have their Physician complete a Health Care Plan. This plan must be completed each school year. Click here for the Individualized Health Care Form.
Transfer Students
Transfer students must show proper documentation of immunizations when registering at ACS. Sixth grade students must show proof of having had the Tdap Booster, which is required by the VDOE and Virginia State Law.
Screenings
Screenings are mandated by the VDOE for all 7th and 10th grade students. Hearing, vision, and Scoliosis screenings will be performed during the months of September and October in the students Physical Education classes. Parents can opt out of the Scoliosis screening by obtaining and completing an opt out form. Please contact Mrs. Wall is you are in need of on opt out form. Click here for the Scoliosis Fact Sheet.

Immunization Schedule
Immunization for the 6th grade Tdap - each Thursday from 8:30-11:00 am and from 1:00-3:30 pm. All other immunizations are given on Wednesdays from 1:30-3:30 pm. No appointment necessary.
